registration/check-in each player will be
required to have the following Medical History/Waiver
Consent form. This form requires parent/guardian
signatures for players under the age of
18. Please
FAX a signed copy of this form to us and
bring the original to the Kamp at check-in.
Players will NOT be able to participate
in the Kamp without this form.
*NOTE: The Medical History/Waiver
Consent form includes a printed version
of the Online Registration form. If you
have completed this form online, you are
not required to complete the printed version.

Who: High
School and College Boys and Girls. Experienced
and 1st time players are
welcome, even whole/part teams.
What to Bring:
Due to the variation in
playing/training surfaces (grass/all-weather/indoor)
available at different venues, players will need
to have rubberized studs (option-running shoes)
or cleats. Recommended items:
2 pairs of socks
2 pairs of shorts
2 Jersey's
Boots with rubber/metal studs
Cross Trainers (Running Shoes)
Mouth Guard/Gum Shield
Water bottle - with a name clearly legible
